A Stanford-led trial found cyclic sighing improved mood and slowed breathing more than meditation did — though not quite in the way most people use it.

There is a reason for the lag. Physiological arousal does not wait for your conclusions. You cannot decide to lower your heart rate, and you cannot reason your way to a calmer nervous system on demand. But breathing sits in an unusual position: it runs automatically and it accepts manual input. That makes it one of the few levers into your own physiology you can actually pull.
In 2023, researchers led by a team at Stanford tested that idea directly, comparing a breathing pattern called cyclic sighing against three other five-minute practices. Below is what the study measured, what it found, what it did not show, and the detail most summaries leave out — which changes how you should actually use this.
Cyclic sighing is a deliberate version of something your body already does on its own every few minutes. A sigh is a double breath: a normal inhale followed by a second, shorter one, then a long exhale. Physiologically it helps reinflate collapsed air sacs in the lungs. In mice, researchers have identified a small population of neurons that generates these sighs automatically, which tells us the pattern is built into the respiratory system rather than being a habit we picked up.
The practice takes that involuntary pattern and repeats it on purpose, with the exhale deliberately extended. The emphasis matters. This is not “deep breathing” in the usual sense — you are not trying to take enormous breaths. You are biasing the cycle toward the exhale.

The study was a remote randomized controlled trial that ran for 28 days, published in Cell Reports Medicine in January 2023 and registered as NCT05304000.
108 people were randomly assigned to one of four groups. Three practised a different five-minute breathing exercise each day — cyclic sighing, box breathing, or cyclic hyperventilation with retention. The fourth practised five minutes of mindfulness meditation, giving the researchers an active comparison rather than a do-nothing control. Everyone was assessed with standard instruments for anxiety, positive and negative affect, and sleep, and wore a wrist tracker that recorded resting heart rate, respiratory rate and heart-rate variability.
Three results stand out.
All four groups, meditation included, reported less state anxiety and less negative feeling by the end of the month. All four improved from their own baselines on several measures.
The breathwork groups, and cyclic sighing in particular, improved positive mood more than the meditation group did. This was a difference in positive affect specifically — feeling more good, rather than only feeling less bad. Those are separable outcomes, and the exhale-focused practice moved the one that is usually harder to shift.
In the breathwork groups, resting breathing rate dropped further than the meditation group’s — a physiological change measured continuously, not a self-report. The people whose breathing slowed the most also tended to report the biggest mood gains. That last point is an association, not a proven mechanism.
Cyclic sighing usually gets presented as an emergency button: you feel stress rising, you do a few sighs, you calm down. That framing is not what the trial tested.
Participants practised five minutes every day for a month. And the effect was not flat across that month — the longer people had been practising, the bigger that daily lift became. The daily positive-affect benefit became larger with more days on the protocol. It looked more like a practice effect than a one-off emergency intervention.
If the strongest evidence is for a daily practice that compounds, then treating cyclic sighing purely as something you reach for mid-crisis is using the weakest version of it.
Using it in the moment is a reasonable extension of the evidence, not a finding of it — the mechanism makes it plausible, and there is little downside to trying, but the trial did not measure whether a one-off session mid-argument changes what you do next.
The better-supported move is the boring one: five minutes a day, whether or not you feel you need it.
This matters, because cyclic sighing has picked up claims the research does not support.
The design has real limits, and the authors say so:
- →It was fully remote. Participants followed video instructions at home, and no one watched to confirm anybody was doing the technique correctly.
- →Adherence was partial. Participants averaged roughly 18 to 20 sessions out of 28 — realistically encouraging, but it means “28 days of practice” describes the assignment, not what happened.
- →It ran four weeks with no follow-up. Whether the benefits persist, fade, or keep growing past a month is unknown.
- →The sample was narrow. 108 people, most recruited from a single undergraduate psychology class at Stanford. That is not a cross-section of stressed adults.
- →Group sizes were small. With 21 to 33 people per arm, the study was underpowered for confident head-to-head ranking between the individual breathing techniques.
- →Physiology came from a consumer wearable. Heart rate, breathing rate and HRV were measured by a wrist-worn tracker rather than laboratory equipment.
None of this makes the finding worthless. It is a registered, randomized, actively controlled trial of a brief, low-cost intervention with a relatively modest burden — that is a useful evidence base for a five-minute habit. It is simply not proof of a physiological override switch.

Sit or lie down somewhere you can breathe without performing for anyone. -

4Exhale slowly through your mouth.
Completely, letting the exhale run longer than the two inhales combined. -
5Repeat without straining.
The exhale should be unhurried, not forced. -
6Continue for five minutes.
That is the dose the study used, so that is the target.
If five minutes feels like too much to start with, three minutes is a reasonable practical starting point — but treat that as a practical accommodation, not a research finding. The trial tested five minutes daily. It did not compare shorter sessions against longer ones, so there is no evidence either way about how three minutes performs.
You are pushing too hard. Ease off the volume of the inhales and lengthen the exhale instead.
The daily session is the practice. Beyond that, the useful moments are transitions — the seams between one state and the next, where your physiology is still carrying the last thing you did:
- →Before a block of focused work or study, so you start settled instead of settling in slowly.
- →Before a difficult conversation, when you want to choose your first sentence rather than discover it.
- →After a stressful event, to stop it bleeding into the next hour.
- →Between work blocks, as a deliberate reset rather than scrolling.
If stress tends to make you stall before starting a task, this is also a useful moment to experiment with the practice. It is the same principle behind designing learning systems that work with your biology rather than against it: change the conditions, and the behaviour gets easier to produce.
Practically, this pairs well with any structured focus routine. Five minutes of cyclic sighing before a focused study block costs almost nothing and changes the state you bring into it. Consistency is the thing that compounds here — the same reason learning how to learn beats collecting individual tactics.

The useful idea underneath cyclic sighing is not that breathing is magic. It is that there is a gap between what happens to you and what you do about it, and that gap is where your physiological state is quietly setting the terms.
You cannot always control the trigger. You can often influence the state you bring into your response. An exhale-focused breathing practice is a small, practical way to influence your physiological state before you respond.
Regulate first. Then decide.
